Our unique on-site care coordination program, Brookdale HealthPlus, is designed to make life easier. The Brookdale HealthPlus Care Manager (RN) can serve as a medical advocate for residents and manage a variety of needs. The Care Manager can coordinate care with residents’ doctors and specialists, organize care transitions, schedule follow up visits, and more. Annual wellness visits, immunizations, and health screenings are also provided.
